ADA / Section 504 Program Complaint Form

ADA/Section 504 of the Rehabilitation Act of 1973 forbids denying qualified individuals with disabilities the opportunity to participate in programs receiving federal financial assistance. Use this form to submit a complaint involving alleged discrimination in access to LKLP programs, services, or activities.
