Taking Care of Your Healthcare-Related Transportation

LKLP is proud to be a provider of non-emergency medical transportation in more than 30 designated Kentucky counties. Eligible citizens can schedule all of their transportation needs to ensure access to healthcare providers. Trips not considered “urgent care” should be scheduled at least 72 hours in advance. However, after hours paging is available, including Sundays and state holidays, for immediate or “urgent care” transportation. Fares are set by Medicaid.

Who is Eligible?

Medicaid, Department For The Blind and Vocational Rehabilitation recipients/participants residing in the following counties:

Region 5: Adair, Allen, Barren, Butler, Edmonson, Green, Hart, Logan, Metcalfe, Simpson, Taylor and Warren counties

Region 13: Breathitt, Clay, Harlan, Jackson, Knott, Lee, Leslie, Letcher, Owsley, Perry and Wolfe counties

Region 15: Bath, Boyd, Carter, Elliot, Greenup, Lawrence, Menifee, Morgan and Rowan counties

How to Schedule

When calling our toll-free number 1-800-245-2826, please be prepared to provide our reservationist with the following information:

  • Name as it appears on your Social Security card (no nicknames), or if you are a Medicaid recipient, the name that is on your medical card
  • Date of travel
  • Address of your pick-up and destination locations. Be specific, including suite and/or building numbers, doctor’s name, etc.
  • Desired arrival time
  • Return time to your place of origin or arrival time to your next appointment/or destination
  • If needed, reserve a seat for a Personal Care Attendant (PCA), child or guest accompanying you
  • If needed, specific details of specialized transportation needed, such as a lift van, etc.
