Creating Community with Compassion

Since 2002, Compassionate Hearts Adult Day Health has been a much-needed source of support for individuals and families in need of supervised care during the day. By providing professional social, health and recreation services in a safe and supervised environment, Compassionate Hearts offers caregivers an alternative to nursing home care for their loved ones. With a focus on promoting independence, we aim to restore the highest level of functioning for each participant and take pride in their successes.

Compassionate Hearts Adult Day Health is staffed by a core group of highly credentialed and trained staff. Our team is equipped to provide participants individualized care plans, including the following health care needs:

An Extended Family


Participants and their families become an extended part of the Compassionate Hearts family. Staff and peers build long-lasting relationships, fostering positive social interactions and peer support. Together, we engage in supervised activities, including arts and crafts, games and outdoor recreation, special events and day trips, good meals and lots of socialization.

What is Adult Day Health?

A program designed to meet the health-related needs of individuals requiring supervised care during the day.

Who is Eligible to Participate?

Clients age 21 years or older with physical, mental or emotional impairments that may need supervision,

skilled nursing, increased social opportunities, assistance with personal care or other daily living activities.

Where are you located?

We’re located at 145 Citizens Lane in Hazard, Kentucky. We serve clients from Perry, Letcher, Breathitt, Knott and Leslie counties.

Is Transportation Provided?

LKLP Community Action and Middle Kentucky River Transportation provide morning and evening transportation to participants. Family members are also welcome to provide their own transportation.

What are your operating hours?

Monday through Friday from 7 a.m. – 5 p.m. ET

What payment is accepted?

HCB Medicaid Waiver programs and self-pay are accepted. Grants may available for qualifying individuals.
